Short-Term Rehab

OVERVIEW

When patients are ready to leave the hospital, but not well enough to go home, they often need continued short-term care to recover completely. That’s where we come in. We work with your physician to set a care plan to meet your goals and abilities with the express purpose of getting you back home.

Our interdisciplinary care team includes:

  • Physicians
  • Registered and Licensed Nurses
  • Rehabilitation Therapists (Physical, Occupational and Speech)
  • Social Workers
  • Registered Dietitians
  • Other Well-Trained Support Staff

CARDIAC REHAB

We provide cardiac rehab to help patients recover from conditions such as congestive heart failure, high attack, and an array of cardiac procedures.

Our goal is to help each patient achieve positive outcomes, regain function and safely return home as quickly as their recovery allows.

Conditions and Services

We treat a variety of conditions and provide services including, but not limited to:p>

  • Recovery from:
  • A recent heart attack
  • Angioplasty or stent placement
  • Pacemaker placement
  • Heart surgery
  • Heart valve replacement
  • Thoracic surgery
  • Abdominal surgery
  • Congestive heart failure
  • Angina
  • Arrhythmias
  • Hypertension

Through our cardiac care, patients have access to the following:

  • Nurses trained in cardiac interventions and management strategies
  • Physical, occupational and speech therapy available up to seven days a week
  • Pain management
  • Progressive exercise regimen tailored to fit each patient's needs and abilities
  • Patient and family education
  • Healthy lifestyle education on matters such as nutrition, smoking cessation, stress and weight reduction
  • Individualized plan of care to meet each patient’s specific needs

IV THERAPY

IV therapy involves the infusion of fluids through an intravenous line. This therapy is used to deliver medications such as antibiotics or to give fluid to treat dehydration or electrolyte imbalance.

ORTHOPEDIC REHAB

We provide quality care for patients with orthopedic injuries or who are recovering from orthopedic procedures.

Our top notch therapy department strives to help each patient achieve positive outcomes and reach his or her highest level of functioning before returning home.

Conditions and Services

We treat a variety of conditions and provide an array of services including, but not limited to:

    • Post-surgical hip, knee, joint replacement/repair, spine surgery or
    • Vertebral fusion
    • Osteoporosis
    • Musculoskeletal disorders including:
    • Degenerative joint disease
    • Osteoarthritis/arthritic
    • Rheumatoid arthritis
    • Orthopedic injury
    • Amputations

    Through our orthopedic care, patients have access to the following:

    • Physical, occupational and speech therapy available up to seven days a week
    • Patient and family education
    • Nutrition assessment and education to promote muscle development and bone strength
    • Progressive exercise regimen tailored to fit each patient’s needs and abilities
    • Individualized plan of care to meet each patient’s specific needs

    POST-SURGICAL REHAB

    After having surgery, one of your top concerns will be your mobility—and how it will impact your daily life. Our dedicated team of experts provides comprehensive care that will help you get back on your feet—and back to your life—more quickly.

    Physical and occupational therapy are crucial to improving your mobility and strength and helping you regain your independence after surgery. Your therapy may begin in the hospital immediately after your surgery, and often must continue in a post-acute center such as Arbors Care Center.

    Our physical & occupational therapy team will personalize a program for you that focuses on the activities you do on a daily basis, including bathing, dressing and walking to the bathroom.

    Getting you home after surgery is our goal!

    PULMONARY REHAB

    As Arbors Care Center’s signature specialty program, our dedicated team of experts provide pulmonary care to help patients breathe easier.

    Our goal is to help each patient achieve positive outcomes, regain function and safely return home as quickly as their recovery allows.

    The interdisciplinary pulmonary care team includes a physician pulmonologist, a respiratory therapist, physical, occupational and speech therapists, registered and licensed nurses, and registered dieticians.

    Conditions and Services

    We treat a variety of conditions and provide an array of services including, but not limited to:

    • COPD – bronchitis, emphysema, bronchiectasis and/or asthma
    • Pneumonia
    • Occupational or environmental lung disease
    • Interstitial fibrosis
    • Lung cancer
    • Post-thoracic and abdominal surgery
    • Disorders requiring CPAP and BiPAP, including obstructive sleep apnea
    • Obesity hypoventilation syndrome
    • Respiratory infection requiring IV antibiotic therapy

    Through our pulmonary care, patients have access to the following:

    • Nurses trained in pulmonary and respiratory interventions and management strategies
    • Physical, occupational and speech therapy available up to seven days a week
    • Anxiety and pain management
    • Progressive exercise regimen tailored to fit each patient’s needs and abilities
    • Patient and family education
    • Healthy lifestyle education such as nutrition education, smoking cessation, stress and weight reduction
    • Individualized plan of care to meet each patient’s specific needs

    RESPITE CARE

    Take Time to Take Care of Yourself

    Approximately 50 million Americans today take care of family members with disabilities and/or chronic illnesses at home. We offer respite care services for caregivers who need a break of a few days or even a few weeks.

    While many families take great pride in caring for their loved ones at home, the physical, emotional and financial consequences can be overwhelming.

    A recent survey found:

    • Caregivers are more likely to report health issues, chronic conditions or a disability than non-caregivers
    • Caregivers who report stress-related issues are more likely to die sooner than non-caregivers

    Respite care provides short-term, temporary nursing care in a home-like setting to family members you typically care for.

    You receive the peace of mind that comes from knowing your family members are being cared for 24 hours a day by a dedicated staff of professionals. And research has shown that providing respite care can have a positive effect on the health of the caregiver.

    Our Respite Care Program includes:

    • An individualized care plan for the resident, including a number of social and recreational activities
    • Physical, occupational and speech-language therapies, as needed
    • Balanced, therapeutic meals, snacks and diet education
    • Activities that include arts and crafts, live entertainment and day trips
    • Religious services

    STROKE CARE

    Our stroke recovery care provides treatment to help patients who have had a stroke return to meaningful roles in life by relearning skills for everyday living. Our goal is to help each patient achieve positive outcomes, regain function and safely return home as quickly as their recovery allows.

    The interdisciplinary stroke recovery care team includes physical, occupational and speech therapists, registered dietitians, registered and licensed nurses, all under the supervision of a physician

    Conditions and Services

    We treat a variety of conditions and provide an array of services including, but not limited to:

    • Stroke
    • Hemorrhaging stroke
    • Transient ischemic attacks (TIAs)

    Through our stroke recovery care, patients have access to the following:

    • Physical, occupational and speech therapy available up to seven days a week
    • Individualized plan of care for each patient’s specific needs
    • Patient and family education
    • Healthy lifestyle programs such as nutrition assessments, smoking cessation, stress and weight reduction
    • Progressive exercise regimen tailored to fit each patient’s needs and abilities

    WOUND CARE

    We provide quality wound care with goals focused on wound healing and prevention of skin care complications. Our mission is to help patients achieve positive outcomes, regain function and safely return home as quickly as their recovery allows.

    The interdisciplinary wound care team includes physical, occupational and speech therapists, registered dietitians and registered nurses.

    Conditions and Services

    • Pressure ulcers
    • Diabetic/neuropathic wounds
    • Venous insufficiency ulcers
    • Arterial insufficiency ulcers
    • Post-surgical wounds, grafts and flaps
    • Infected and/or draining wounds requiring IV antibiotics
    • Wounds requiring frequent dressing changes
    • Amputations
    • Post-traumatic wounds

    Through our wound care services, patients have access to the following:

    • Completion of functional and nutritional assessments
    • Use of special mattresses and bed surfaces
    • Healthy lifestyle education on topics such as nutrition education, smoking cessation, stress and weight reduction
    • Physical, occupational and speech therapy available up to seven days a week
    • Progressive exercise regimen tailored to each patient’s needs and abilities
    • Patient and family education on wound healing and prevention, including; specific diabetic education
    • Individualized plan of care for the patient’s specific needs
